Cottage Grove Place is recruiting for a hospitality focused Health & Wellness Navigator to join our team!

The Health & Wellness Navigator is responsible for building relationships, solving problems, and locating resources for residents transitioning throughout the continuum of care and to their home. The Health and Wellness Navigator will evaluate residents’ needs and assist them in accessing the available resources needed to ensure a seamless transition between appropriate levels of care offered at the community. This position will also assist residents in facilitating their wellness and healthcare needs while ensuring social integration into the community. Responsible for coordinating the programs designed to meet these needs and to develop future programs. The ultimate goal is to guide residents, family members and/or caregivers through successful health and wellness transitions to achieve the optimal level of wellbeing and appropriate level of care. The Health & Wellness Navigator will facilitate communication with all key resources and stakeholders.

Supervisory Responsibility: Train, supervise, and provide feedback on tasks performed by nursing staff in Choice In-Home Services (CIS).
 

Here are a few of the daily responsibilities of a Health & Wellness Navigator:

  • Ensure cross-functional departmental support of all post-acute services within the community.
  • Ensure residents are in the appropriate levels of care (Independent, Assisted, Memory Care, Respite, Skilled Nursing and Rehab) within the community and are receiving the supportive services needed to obtain optimal levels of health.
  • Collaborates with other members of the community team in identifying and recommending additional services or transitions within the continuum of care for residents with changing needs.
  • Strives to achieve high levels of resident satisfaction.
  • Coordinates and/or attends relative community meetings related to resident transitions/status updated, such as Interdisciplinary Team Level of Care Meeting, Daily Health Center Stand Up Meeting, Care Coordination Meeting, Weekly Risk Meeting, Independent Living Resident Health Committee, Support Groups, and Director’s Meetings.
  • Maintains awareness and promotes all internal services to promote resident retention.
  • Coordinates new resident orientation by encouraging participation in community programs and engagement in social events.
  • Continuous evaluation of the physical, emotional and/or social needs of residents within the community.
  • Serve as resident liaison, including coordinating assessments, offering consultations, and aiding with coordination of both internal and external resources.
  • Interacts with the resident and family members when a change in the resident’s condition necessitates additional services or a physical move within the continuum of care.

Here are a few of the qualifications we need you to have:

Required:

  • Minimum Bachelor's degree in Social Work or Health & Wellness, or related field.
  • Minimum of three to five (3-5) years of organizational or management experience supervising others.
  • Able to manage confidential information in an appropriate and discreet manner. Must act in accordance with Resident Bill of Rights.
  • Possess excellent written, verbal and listening communication skills. 
  • Possess computer and word processing skills.

Preferred:

  • Minimum Registered Nurse (RN) degree from accredited school of nursing program.
  • Currently registered with the State of Iowa Board of Nurse Examiners.
  • Minimum three to five (3-5) years’ experience working in long-term care, post-acute care, or other health care setting.
  • Strong interpersonal skills including the ability to motivate and encourage residents to achieve maximum independence and quality of life.
  • Able to problem solve, make ethical recommendations, and define appropriate boundaries.
  • Effective organizational skills including delegation, managing multiple priorities, time management and problem solving.

Salary Range: $65,000 - $75,000/year
